Our multidisciplinary team includes physicians, advance practice nurses, registered nurses, social workers. Together, we provide an extra layer of support for people living with serious illness. Our palliative care services provide relief from symptoms, pain, and stress as well as emotional support.
Our Mission
We aim to increase awareness of palliative care in the community. We aim to improve healthcare literacy for patients and their loved ones, assist patients in pursuing their goals and wishes, and improve quality of life quality of life for those with serious illness.

Our Services
Our team provides palliative care services in the hospital setting. During your hospital stay, our physicians and nurse practitioners provide undivided attention in discussing your condition, treatment plan, and healthcare goals. We also provide expert symptom management. Upon discharge, we offer virtual follow-up care with an advanced practice nurse as well as counseling services with a licensed medical social worker. In addition, our nurse case manager is available by phone or video to assist with healthcare coordination or resolution of urgent concerns.
